1. Enrollment Documents: All enrollment documents must be picked up at the preschool office.
2. Registration Fee: This is an annual $100 non-refundable fee and is due with your Enrollment Form to reserve your child’s position in the program.
3. Annual Materials Fee: $250.00
4. Health Records: Mantoux TB test and clearance must be done prior to entry in school and be no older than one year. No admittance without a current TB clearance.
5. Physical: Children must have a physical completed by the family doctor within a year prior to school entry. This should be turned in by the first day of school.
6. Immunizations: A current record of all immunizations on State of Hawaii Form 14 is preferred. See director if you need a form for your doctor. Without the above health requirements or signed immunization waiver form, we are required to terminate enrollment.
